DPCA sees patients age 2 and older, including well child checks. We do not see patients under 2.

Membership includes care delivered by DPCA. It is not insurance. We recommend every member carry coverage for major medical events, and Momentum Benefits can help you find the right plan.

One membership, no tiers. $89 a month for an adult, $35 for a child on a family account, a $248 family maximum for 2 adults and 2+ children, plus a one time $75 enrollment fee per family.
